Emergency Call Reports
Patients: To request a copy of your report, you must present photo identification in person at McLane - Black Lake Fire Department Station 91, 125 Delphi Rd NW., Olympia WA 98502.
Attorneys and other agencies: To request a report on behalf of a client, submit a written request on letterhead along with authorization to release medical information signed by your client. There is a $5.00 fee for Non-Patient Requests. Requests can be submitted by mail to:
